Submitting an appointment request does not guarantee an appointment. Our office will contact you to confirm scheduling.
We use Remedly for electronic patient intake, electronic health records, patient portal communications, internal practice messaging, insurance billing, and patient billing.
Please avoid submitting sensitive medical or financial information through general website contact forms. Our staff may direct you to an appropriate patient intake system or another communication method.
Use of third-party patient portal services may be subject to additional applicable terms and privacy protections.
Advanced Eye Prosthetics uses Weave to provide optional text messaging communications to patients who have provided appropriate consent.
TYPES OF MESSAGES
Messages may include appointment reminders, scheduling confirmations, rescheduling information, administrative updates, and responses to patient inquiries.
HOW TO OPT IN
Patients may choose to receive text messages by selecting the optional text messaging consent checkbox in their electronic patient intake documents through Remedly.
Our website does not currently provide a separate text messaging enrollment form.
Providing a telephone number through a website contact form or appointment request does not automatically enroll you in optional text messaging.
Consent to optional text messaging is not required to receive treatment or services from Advanced Eye Prosthetics.

Message frequency varies based on appointment schedules, patient requests, and administrative needs.
Message and data rates may apply.
OPTING OUT AND GETTING HELP
You may opt out of optional text messages at any time by replying STOP or contacting our office at 833-208-9009.

Wireless carriers are not liable for delayed or undelivered messages.
